Concept Rehab, Inc in Oregon, OH is seeking a Full-Time Physical Therapist to evaluate and treat patients in a post-acute setting, develop individualized plans, and document care.

You will collaborate with an interdisciplinary team, educate patients, and help them regain independence while benefiting from a supportive workplace and mentorship.

Qualifications

• Licensed Physical Therapist (PT) in Ohio or eligible for Ohio licensure.
• New graduates are encouraged to apply.
• Strong communication and interpersonal skills.
• Ability to work collaboratively with an interdisciplinary team.
• Passion for patient-centered care.

Responsabilités

• Evaluate and treat patients in a post-acute/skilled nursing setting.
• Develop individualized treatment plans focused on mobility, strength, balance, and independence.
• Collaborate with Occupational Therapists, Speech-Language Pathologists, nursing staff, physicians, and families.
• Complete accurate, timely documentation.
• Educate patients and caregivers on treatment plans and home exercise programs.
• Help patients achieve meaningful functional outcomes.
