Job summary

Community Skilled Health Care Centre in Warren, OH is seeking a full-time Physical Therapist to join our interdisciplinary rehab team. You will help residents regain strength, mobility, and independence so they can enjoy the best quality of life possible.

Evaluate patients, develop plans, provide therapy, and collaborate with nursing and therapy colleagues. New grads welcome; Ohio PT license required. Competitive pay and full benefits, with flexible scheduling where possible.

Qualifications

• Ohio PT license in good standing is required.
• Experience in a skilled nursing facility or rehab setting preferred, but new grads are welcome.
• Strong communication and teamwork in an interdisciplinary rehab team.

Responsibilities

• Provide direct therapy services in accordance with physician orders.
• Collaborate with nursing, OT, and SLP teams.
• Educate patients, families, and caregivers on treatment plans and home exercises.
