Job summary

Total Care Therapy LLC in Westerville, OH is seeking a compassionate Physical Therapist to evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

In this full-time, on-site role you will deliver skilled PT services, supervise PTAs, complete timely EMR documentation, educate residents and staff, and collaborate with an interdisciplinary team across assisted living and independent living

Qualifications

• Current State Physical Therapist (PT) License.
• CPR/BLS Certification required.
• Valid driver's license and reliable transportation.
• Experience in geriatrics/home health/assisted living preferred.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including exercise and mobility training.
• Reassess progress and modify treatment plans as appropriate.
• Supervise PTAs in accordance with state regulations.
• Document using the EMR in a timely and accurate manner.
• Educate residents, caregivers, and staff on treatment plans and home programs.
• Collaborate with interdisciplinary team for coordinated care.
• Maintain a productive clinical caseload while delivering quality therapy.

About the Role We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.

Qualifications

• Current State Physical Therapist (PT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
