Job summary

Premier Therapy LLC in Wooster, Ohio is hiring a Full-Time Physical Therapist (PT) to join a collaborative team focused on improving mobility and quality of life. You will evaluate patients, develop plans of care, provide skilled therapy, monitor progress, set functional goals, educate patients and families, and document care in coordination with the interdisciplinary team.

Required qualifications include a degree from an accredited Physical Therapy program and a valid PT license or eligibility

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current PT license or eligibility for licensure in the state of practice.
• SNF/long-term care experience preferred, but not required.

Responsibilities

• Evaluate patients and develop individualized plans of care.
• Provide skilled physical therapy treatment and monitor patient progress.
• Establish functional goals and modify treatment plans as appropriate.
• Educate patients, families, and caregivers.
