Job summary

Garden City Health Care Center in Modesto, CA is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team in a skilled nursing facility setting. The PT will evaluate and treat residents, collaborate with families and physicians, and document care accurately to support resident independence.

This full-time role requires CA licensure, a BS in PT (MS/PhD preferred), and the ability to supervise PTAs and aides.

Qualifications

• California PT license in good standing is required and CE must be kept up-to-date.

We support our residents in achieving their highest level of independence and quality of life through patient-centered, evidence-based care.Whether you are an experienced PT or early in your career, we offer a **supportive, collaborative environment** where your expertise and growth are valued.### **General Purpose**The **Physical Therapist** evaluates and treats residents, communicates with families, physicians, and interdisciplinary team members, and maintains accurate clinical documentation. The PT also provides education and in-services to facility staff to support optimal resident outcomes.### **Essential Duties and Responsibilities*** Evaluate residents within **24 hours (or one business day)** of physician referral; document delays appropriately* Develop individualized treatment plans and obtain physician approval* Treat residents according to physician-approved plans of care* Supervise Physical Therapy Assistants (PTAs), aides, and students in compliance with state practice act* Assist nursing staff with training of Restorative Aides* Communicate resident progress, challenges, and plans with rehab and interdisciplinary teams* Participate in patient care conferences, utilization review, and rehab meetings as needed* Complete evaluations, daily treatment notes, weekly progress notes, recertifications, and discharge documentation per regulatory and payer requirements* Educate residents, families, and caregivers on treatment plans, maintenance programs, and discharge preparation* Participate in discharge planning and care coordination* Recommend and facilitate ordering of durable medical equipment (DME) to promote functional independence* Report equipment issues to ensure safe and effective operation* Maintain positive, professional relationships with residents, families, staff, and colleagues### **Supervisory Responsibilities*** Supervise **Physical Therapy Assistants, aides, and students**### **Qualifications****Education & Experience*** Bachelor’s degree in Physical Therapy required* Master’s or Doctorate degree in Physical Therapy preferred* Skilled nursing or geriatric experience preferred**Licensure & Certifications*** Current, active **California Physical Therapist license** in good standing* Must meet all continuing education requirements per state regulations**Skills*** Proficient in electronic documentation systems (**Casamba and PointClickCare preferred**)* Strong clinical judgment, communication, and organizational skills* Ability to work independently and collaboratively### **Physical Demands*** Frequent standing, walking, reaching, pushing, and pulling* Ability to exert up to **50 pounds frequently** and **100 pounds occasionally*** Occasional climbing, balancing, stooping, kneeling, crouching, or crawling* Visual abilities include close, distance, color, peripheral, and depth perception* Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions### **Work Environment*** Skilled nursing and rehabilitation setting* Noise level typically low to moderate* Reasonable accommodations available### **Additional Information**This job description does not restrict management’s right to assign or reassign duties at any time.
