Job summary

Garden City Health Care Center in Modesto, CA is hiring a full-time Physical Therapist (PT) for a skilled nursing facility. Qualifications

Responsibilities

• Evaluate residents within 24 hours of physician referral and document delays.
• Develop individualized treatment plans with physician approval.
• Supervise PTAs, aides, and students in compliance with state practice act.
• Educate residents, families, and caregivers on treatment plans and discharge prep.
• Participate in discharge planning and care coordination.
• Complete evaluations, daily treatment notes, weekly progress notes, and discharge documentation.
• Report equipment issues to ensure safe operation.
