Job summary

Rose Mountain Care Center in New Jersey is seeking a dedicated Full Time Physical Therapist to join our rehabilitation team in a skilled nursing facility. You will evaluate and treat residents with orthopedic, neurological, and post-surgical needs to improve mobility and independence.

We welcome new grads with excellent mentorship; strong communication and teamwork are essential as you collaborate with physicians, nurses, and other therapists.

Qualifications

• Active Physical Therapist license in New Jersey or eligibility.
• New grads welcome with mentorship and training.
• Experience in SNF/geriatrics is a plus.
• Strong communication and interpersonal skills.
• Ability to work in an interdisciplinary team.
• Compassionate, positive attitude.

Responsibilities

• Evaluate and treat residents in subacute and long-term care settings.
• Develop personalized treatment plans to improve mobility.
• Collaborate with physicians, nurses, and other therapists.
• Document progress in the EMR.
• Participate in care plan meetings and family conferences.
• Provide compassionate, patient-centered care.
• Supervise PTAs as needed.
• Report to the Director of Rehabilitation.

About the Role

We are seeking a passionate and dedicated Full Time Physical Therapist to join our dynamic rehabilitation team in our skilled nursing facility in New Jersey. If you love helping seniors regain their mobility, independence, and quality of life, this is the perfect opportunity for you!

What You’ll Do

• Evaluate and treat residents in subacute and long term care setting with a variety of orthopedic, neurological, and post-surgical conditions
• Develop personalized treatment plans to improve strength, balance, gait, and functional mobility
• Collaborate closely with physicians, nurses, occupational therapists, and speech therapists
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a supportive team environment
• Supervise Physical Therapist Assistants (PTAs) as needed
• Report directly to the Director of Rehabilitation

What We’re Looking For

• Active Physical Therapist license in the state of New Jersey (or eligible to obtain)
