Human Touch Home Health seeks licensed Physical Therapists (PTs) to provide in-home care across Floyd, Giles, and Pulaski Counties. We offer Full-Time and PRN opportunities with a $5,000 sign-on for full-time and $2,500 for PRN, plus mileage reimbursement and comprehensive insurance options.

Qualifications

• Active Virginia Physical Therapist License is required.
• Graduate of an accredited Physical Therapy program.
• Valid Driver's License and reliable transportation.
• Strong clinical, communication, and documentation skills.
• Ability to work independently in patients' homes.
• Home Health and OASIS experience preferred, but not required.

Responsibilities

• Complete PT evaluations in patients' homes.
• Develop and manage individualized plans of care.
• Provide therapeutic exercise, gait, balance, transfer, and mobility training.
• Assess home safety and fall risks.
• Educate patients and caregivers.
• Monitor progress and adjust treatment plans.
• Coordinate care with the interdisciplinary team.
• Complete timely documentation using WellSky.
• Manage patients from evaluation through discharge.

Service Area

Patients are located throughout Floyd, Giles, and Pulaski Counties, including Floyd, Pearisburg, Narrows, Pembroke, Rich Creek, Pulaski, Dublin, Fairlawn, and surrounding communities.

You do not have to cover every county. Assignments can be coordinated based on patient needs, your availability, and the areas you're comfortable serving.
