Job summary

Human Touch Home Health in Virginia is seeking a Physical Therapist (PT) for PRN home health opportunities across Prince William County and surrounding communities. You can select areas that fit your schedule and build your caseload accordingly.

Responsibilities include conducting in-home PT evaluations, developing individualized plans of care, providing therapeutic exercise, gait and balance training, and coordinating with a care team.

Qualifications

• Active Virginia Physical Therapist license is required.
• Graduate of an accredited Physical Therapy program.
• Valid Driver's License and reliable transportation.
• Strong clinical assessment and communication skills.
• Ability to work independently in patients' homes.
• Home Health Experience preferred, but not required.
• WellSky experience is a plus.

Responsibilities

• Complete physical therapy evaluations in patients' homes.
• Develop individualized plans of care.
• Provide therapeutic exercise, gait, balance, transfer, and mobility training.
• Help patients improve strength, endurance, and functional independence.
• Assess home safety and identify fall risks.
• Educate patients and caregivers on mobility, safety, and home exercise programs.
• Monitor patient progress and adjust treatment plans as needed.
• Complete reassessments and discharge planning.
• Coordinate care with physicians, nurses, therapists, and other team members.
• Complete accurate and timely documentation using WellSky.

Human Touch Home Health is seeking a Physical Therapist (PT) for PRN home health opportunities throughout Prince William County, Virginia.

Patient opportunities may include Manassas, Manassas Park, Woodbridge, Dale City, Lake Ridge, Gainesville, Haymarket, Bristow, Dumfries, Nokesville, Triangle, Quantico, and surrounding communities.

You do not have to cover the entire county. Choose the areas that work best for you and build your caseload around your availability.
