Job summary

Woburn Rehabilitation and Nursing Center in Massachusetts is seeking a compassionate Physical Therapist to join our therapy team. We offer Full-Time, Part-Time, and PRN opportunities for clinicians who help patients improve mobility and independence.

Responsibilities include evaluating patients, creating individualized treatment plans, collaborating with healthcare professionals, documenting progress, and educating patients and families to promote safety and quality of life.

Qualifications

• Massachusetts state license required.
• Ability to collaborate with a dynamic interdisciplinary team.
• Strong evaluation and clinical reasoning skills.
• Ability to develop individualized treatment plans.
• Strong communication with patients, families, and staff.
• Ability to educate and counsel patients and families.

Responsibilities

• Provide physical therapy services to promote rehabilitation and quality outcomes.
• Evaluate and assess patients according to state payer and facility requirements.
• Develop and implement individualized treatment plans with measurable goals.
• Collaborate with other healthcare professionals to optimize patient outcomes.
• Communicate effectively with staff, patients, and families for coordinated care.
• Document treatment plans, goals, and progress per Medicare and payer guidelines.
• Participate in family conferences and provide progress updates as needed.
• Complete documentation and treatment charges in a timely manner.
• Promote safety, independence, dignity, and quality of life during rehab.
