Job summary

Nightingale Home Healthcare of Illinois Inc is seeking part-time Physical Therapists to join our team in the Chicago area. You will deliver skilled therapy, educate families, and collaborate with physicians to ensure high-quality home health care.

Qualifications

• Active Physical Therapist license required.
• Minimum 1 year PT experience (Home Health/OASIS preferred).
• WellSky EMR experience a plus.
• Must have TB test within last 12 months.
• Current CPR certification.
• Valid driver’s license & reliable transportation.
• Must be available to attend orientation at our Willowbrook office.
• Available Monday–Friday, 8 AM–5 PM.

Responsibilities

• Evaluate patients within 72 hours of receiving a physician referral.
• Develop and implement individualized Plans of Care.
• Provide therapy services and supervise PTAs and HHAs.
• Communicate effectively with physicians and the interdisciplinary team.
• Maintain accurate, timely documentation in EMR.
• Evaluate treatment effectiveness and update care plans as needed.
• Participate in interdisciplinary team meetings and continuing education.
• Maintain patient confidentiality per HIPAA standards.
