Job summary

Loyola Medicine Home Care and Hospice is seeking a full-time Home Care Physical Therapist to deliver patient-centered physical therapy in patients' homes. You will assess, plan, and implement individualized care in collaboration with a multidisciplinary team.

Role requires Illinois PT licensure and a degree in physical therapy; experience in home health care is preferred. The position emphasizes autonomous work, technology use, and ongoing professional development.

Minimum Qualifications:

• Bachelor’s degree with progressive experience or Master’s degree


• Licensed to practice as a physical therapist in Illinois


• Current working knowledge of conditions of participation and home healthcare experience preferred. Must have current driver’s license and reliable transportation to and from work site.
