Job summary

Nightingale Home Healthcare is seeking part-time Physical Therapists to deliver skilled therapy to home health patients in the Chicago area. You will teach family members proper care techniques and collaborate with the care team and physicians to optimize outcomes.

Qualifications

• Active Physical Therapist license.
• Minimum 1 year of PT experience (Home Health/OASIS preferred).
• Current TB test within 12 months.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Must be able to attend orientation at Willowbrook office.

Responsibilities

• Evaluate patients within 72 hours of receiving a physician referral.
• Develop and implement individualized Plans of Care.
• Provide therapy services and supervise PTAs and HHAs.
• Communicate effectively with physicians and the interdisciplinary team.
• Maintain accurate, timely documentation in EMR.
• Evaluate treatment effectiveness and update care plans as needed.
• Participate in interdisciplinary team meetings and continuing education.

• WellSky (Kinnser) EMR experience a plus


• Physical (within 12months of first patient contact)


• Current TB test (within last 12 months)


• Current CPR certification


• Valid driver’s license & reliable transportation


• Must be available to attend orientation at our Willowbrook office


• Available Monday–Friday, 8 AM–5 PM
