Job summary

One Home Health, a Medicare-certified home health agency serving the Chicago area, seeks a compassionate Home Health Physical Therapist (PT) to join as a part-time clinician. You will deliver one-on-one rehabilitation in patients' homes, helping them regain mobility and independence with individualized plans and interdisciplinary collaboration.

Qualifications

• Current Illinois Physical Therapist license in good standing.
• Doctor of Physical Therapy (DPT) degree.
• Valid driver's license and reliable transportation.
• Strong communication, organization, and documentation skills.

Responsibilities

• Conduct comprehensive in-home physical therapy evaluations and assessments.
• Develop, implement, and update individualized treatment plans.
• Provide therapeutic interventions to improve mobility, strength, balance, and functional independence.
• Monitor patient progress and modify treatment plans as appropriate.
• Educate patients and caregivers on exercises, safety techniques, mobility strategies, and injury prevention.
• Accurately document visits and patient outcomes in accordance with agency standards and regulatory requirements.
• Communicate patient progress and care recommendations with physicians and the interdisciplinary healthcare team.
• Maintain compliance with Medicare, state, agency, and professional practice standards.

Position Summary

As a Home Health Physical Therapist, you will provide skilled rehabilitation services to patients in their homes, helping them improve mobility, restore function, and achieve their highest level of independence.
