Spring Creek Rehabilitation and Nursing Center is seeking a skilled Physical Therapist (PT) for a full-time position at our Harrisburg location. The role includes delivering individual therapy per care plans and collaborating with the care team.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unencumbered Pennsylvania Physical Therapist license.
• Experience in hospital, nursing facility, or related medical setting preferred.
• Ability to plan, organize, and interpret physical therapy services.

Responsibilities

• Provide individual therapy to residents per care plans.
• Assist in resident assessments and therapy plan development with MDS.
• Adjust therapy plans to support resident care goals.
• Brief residents on procedures and prepare equipment for therapy.
• Participate in discharge planning and care plan development.
• Interview residents and families as needed; coordinate with departments.
• Maintain therapy area safety and equipment, and communicate resident changes.

Requirements of the Physical Therapist (PT):

• Must possess, as a minimum, a Bachelor’s Degree in Physical Therapy from an accredited college or university.
• Must have, experience in a therapist capacity in a hospital, nursing care facility, or other related medical facility.
• Musthave training in rehabilitative and restorative therapy practices.
• Must possess a current, unencumbered license to practice as a Physical Therapist in this state
• Must be knowledgeable of physical therapy procedures
• Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise otherpersonnel.
• Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies,procedures, etc., of the physical therapy service
• Must maintain the care and use of supplies, equipment, etc., and maintain the appearance of therapy areas.
• Must be able to relate information concerning a resident’s condition
• Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.

Responsibilities of the Physical Therapist (PT):

• Providing individual therapy services to residents according the care plans.
• Assisting in resident assessment activities as needed
• Assisting as needed in developing therapy plans for individual residents in conjunction with MDS.
• Keeping the Therapist informed of any need to adjust therapy plans to match or support resident care plan goals.
• Briefing residents in procedures involved in physical therapy and preparing necessary equipment.
• Participating in discharge planning, development and implementation of resident care plans, resident assessments, etc.
• Interviewing residents and family members as necessary.
• Coordinating therapy appointments with other departments.
• Following therapy department safety standards.
