Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services in accordance with physician orders and established plans of care.
• Assess mobility, balance, strength, endurance, and functional performance.
• Educate residents, families, and caregivers regarding treatment goals and home exercise programs.
• Monitor patient progress and adjust interventions as appropriate.
• Complete accurate and timely documentation in accordance with company policies and regulatory requirements.
• Communicate effectively with physicians, nursing staff, and interdisciplinary team members.
• Maintain productivity standards while delivering quality patient care.
