Job summary

Cedar Point Care & Rehabilitation in Clyde, OH seeks a dedicated Physical Therapist to lead rehabilitative care for residents in a skilled nursing setting. Responsibilities

  • Screen/evaluate patients with cognitive and/or ADL disorders and develop care plans
  • Utilize standardized assessments and evidence-based practice to guide interventions
  • Provide rehabilitative, skilled treatment per regulatory requirements
  • Administer therapies including ADL retraining, therapeutic activities, exercises, modalities
  • Assess progress and adjust treatments in the clinical record
  • Provide discharge summaries and coordinate with families and care teams
  • Supervise PTA, aides and students when applicable
  • Maintain documentation in electronic records per company policy

JOB SUMMARY

The Physical Therapist is responsible for providing rehabilitative therapy to patients upon referral for cognitive and/or ADL physical functioning disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.

This position reports to the Director of Rehabilitation Services.

JOB DUTIES
  • Effectively screens/evaluates patients and/ with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
  • Utilizes standardized assessments and evidence-based practice to support clinical interventions.
  • Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
  • Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
  • Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
  • Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the patients’ physical disorders.
  • Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
  • Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
  • Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy.
  • Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
  • Participation in required meetings including but not limited to: patient care conferences, utilization review meetings, rehabilitation meetings, etc.
  • Participate in facility and company required trainings, in-services and conferences.
  • Able to supervive and support physical therapist assistants; rehabilitation aides and/or students.
  • Adherence to State Required practice acts, code of conduct, ethics during professional practice as an Physical Therapist.
  • Adherence to Company requires policy, procedures and code of conduct.
  • Active State Licensure

  • Bachelor's degree or higher in Physical Therapy or related field
  • Active state license
