Job summary

Shores of Worthington is seeking a PRN In-House Physical Therapist (PT) to join our residential health care facility in Worthington, MN. The PT will provide direct patient care, assess rehabilitation needs, and develop individualized treatment plans in collaboration with the physician and interdisciplinary team.

Qualified candidates will have an accredited Physical Therapy degree and MN license, plus experience in geriatric or long-term care, EMR documentation, and a patient-centered,

Qualifications

• Graduated from an accredited Physical Therapy program and licensed to practice in MN.
• Experience with long-term care and geriatric populations preferred.
• Strong EMR documentation skills and interdisciplinary collaboration.

Responsibilities

• Assess referred patients and provide direct PT care to determine rehabilitation needs.
• Develop and administer individualized treatment plans with goals aligned to patient capacity.
• Document patient progress and educate patients and families on plan of care.

Shores of Worthington is looking for a PRN In- House Physical Therapist (PT) candidate to become a part of our team!

Responsibilities Include:

• The Physical Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation.
• The Physical Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
• The Physical Therapist will set realistic and achievable goals for their patients, document and record the patient's condition, and educate patients and families with regard to the plan of care.

Qualifications/Requirements:

• Graduated from an accredited Physical Therapy college program or university
• Valid professional license and registration for the state of MN
• Experience with long-term care and/or geriatric care is preferred but not limited to
• Well-versed with EMR documentation Evaluations, weekly progress notes, recertification, and discharge documentation
• Collaborates with all disciplines to provide the best patient-centered treatment and care
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Adhere to scheduled hours and demonstrate professionalism at all times
• Perform all other duties assigned by the Director of Rehabilitation Compliance with confidentiality & HIPAA
