Job summary

Shores of Worthington is seeking a PRN In-House Physical Therapist to join our rehabilitation team. The PT will assess referred residents, develop and oversee treatment plans, and collaborate with an interdisciplinary team in a geriatric care setting.

The role requires MN licensure, an accredited PT degree, and strong documentation skills within EMR systems. PRN status implies flexible on-site hours with commitment to quality patient-centered care and HIPAA compliance.

Qualifications

• Graduated from an accredited Physical Therapy college program or university.
• Valid professional license and registration for the state of MN.
• Experience with long-term care and/or geriatric care preferred.
• Well-versed with EMR documentation: evaluations, weekly progress notes, recertification, discharge documentation.
• Collaborates with all disciplines to provide the best patient-centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence.
• Adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by the Director of Rehabilitation; HIPAA compliance.

Responsibilities

• Assess referred patients requiring rehabilitation services, including direct patient care and evaluating medical condition, abilities, and rehab potential.
• Establish and administer a treatment program with goals aligned to patient capacity and physician direction.
• Set realistic goals, document patient condition, and educate patients and families about the care plan.

Shores of Worthington is looking for a PRN In- House Physical Therapist (PT) candidate to become a part of our team!

Responsibilities Include:

• The Physical Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation.
• The Physical Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
• The Physical Therapist will set realistic and achievable goals for their patients, document and record the patient's condition, and educate patients and families with regard to the plan of care.
