Job summary

Shores of Worthington is seeking a PRN Physical Therapist Assistant to provide rehabilitation services to residents in a residential health care setting in Worthington, Minnesota.

Responsibilities include delivering therapy under the supervision of a PT, maintaining documentation, coordinating with the rehab team, and supporting discharge planning. New grads are welcome and a sign-on bonus is offered for full-time or part-time roles.

Qualifications

• Associate's degree from an accredited PTA program.
• State licensure or PTA certification.
• Strong communication and interpersonal skills with residents, families, and healthcare team.
• Compassion, patience, and empathy in long-term care.
• Ability to adapt treatment plans based on patient progress.
• Knowledge of anatomy, physiology, and therapeutic exercise.
• Familiarity with electronic medical record systems and documentation practices.

• Maintain a neat, organized therapy department.
• Assist with admin duties for therapy services.
• Identify new rehab admissions and notify PT when appropriate.
• Communicate upcoming discharges with Facility staff.
• Attend care plan meetings when requested.

Shores of Worthington is looking to hire an In-House PRN Physical Therapist Assistant to provide rehabilitation services to our residents!

Primary Job Responsibilities:

• Maintain the standards for rehabilitation services in accordance with the Company’s Mission and Goals

• Be courteous to all patients and respectful of them, their environment, and their culture

• Provide quality therapy services at every patient contact, including treatment interventions, program implementation, discharge planning, and related documentation and communication under the direction and supervision of the physical therapist

• Prioritize and maintain a work schedule that is appropriate for the facility and in accordance with your employment

• Assist Therapy management staff in maintaining a positive, productive, and beneficial working relationship with all Facility staff

• Adhere to the policies and procedures necessary for the day-to-day operations of the rehab department

• Screen patients to determine the need for evaluation under the supervision of the PT

• Treat patients to relieve pain and develop or restore function to achieve maximum performance under the supervision of a physical therapist

• Monitor patient response to intervention, communicate any changes, and follow through with all updates to the treatment modalities, goals, and frequency as needed under the direction and supervision of the physical therapist

• Fulfills documentation requirements in a complete, appropriate, and timely manner

• Communicate and collaborate with other team members regarding the care of clients receiving therapy services

• Assist with data collection on patients’ progress under the supervising PT

• Plan for patient's discharge when maximum benefits have been met in coordination with the supervising PT

• Provide complete and correct billing summaries as required

• Attend in-service education when provided to rehab staff

• Maintain a neat and well-organized therapy department

• Perform administrative duties to assist in the provision of therapy services

• Identify all new rehab admissions and notify the physical therapist when appropriate

• Communicate upcoming patient discharges with Facility staff as required

• Attend appropriate care plan meetings when requested

• Provide other job-related tasks and duties as assigned

Requirements:

• Associate's degree from an accredited Physical Therapy Assistant program

• State licensure or certification as a Physical Therapy Assistant.
