Synchrony Rehab is seeking a licensed Physical Therapist (PT) to join our senior living communities in Winston-Salem, NC. The role offers a Monday–Friday schedule with no weekends or holidays, providing comprehensive PT services across multiple facilities including The Southwood, Southfork, Somerset Court of Mocksville, and Mocksville Senior Living.

Qualifications

• Must have and maintain a current state PT licensure.
• Current CPR certification preferred.
• 0-1 years of experience recommended.
• Degree in Physical Therapy from an accredited program.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehab needs.
• Develop and implement personalized treatment plans including exercises, manual therapy, and modalities.
• Educate patients and families on exercise techniques, posture, and body mechanics.
• Maintain accurate documentation of progress and treatment plans per facility policies and regulatory requirements.
• Collaborate with the interdisciplinary team to ensure comprehensive patient care.
• Perform other duties as assigned.

Job Summary

The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices and ethical standards.

Licenses and Certifications
