Job summary

Synchrony Rehab is seeking a Physical Therapist to provide a full range of PT services, including assessment, planning, and therapeutic interventions in an interdisciplinary environment.

You will assess patients with diverse rehab needs, create personalized treatment plans, educate patients and families, and document progress in accordance with facility policies. A current PT license and CPR are required or preferred, with 0–1 years of experience common.

Qualifications

• Must hold a current, valid state PT license or certification.
• Current CPR certification, preferred.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises and modalities.
• Educate patients and families on exercise techniques, posture, body mechanics, and strategies to manage conditions.
• Maintain accurate documentation of patient progress and outcomes in line with facility policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
