Job summary

The Springs at Wyandot Trail is seeking a Licensed Physical Therapist to deliver comprehensive PT services in a collaborative care setting. You will assess, plan, and implement therapeutic interventions, working with an interdisciplinary team to optimize patient outcomes.

Responsibilities include patient education, documentation, and adherence to regulatory standards. CPR certification preferred; prior experience is a plus. This is a full-time on-site role at our Ohio facility.

Qualifications

• Must have current, valid state PT licensure or certification.
• Current CPR certification preferred.
• Education: Doctor of Physical Therapy (DPT) from an accredited program; Experience: 0-1 years.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
