Job summary

Tice Valley Post Acute is seeking a compassionate Physical Therapist to join our rehabilitation team in Walnut Creek, CA. We are hiring for Full-Time, Part-Time, and PRN roles with pay rate of $60–$65 per hour, depending on experience.

The PT will evaluate and treat residents, develop individualized treatment plans, monitor progress, and collaborate with physicians, nursing staff, families, and the interdisciplinary team. The role includes staff education and ensuring compliant documentation.

Qualifications

  • Licensed Physical Therapist in California.
  • Ability to evaluate and treat residents per physician orders.

Responsibilities

  • Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies.
  • Develop, implement, and update individualized treatment plans to improve mobility, strength, balance, endurance, and functional independence.
  • Provide skilled physical therapy services and monitor resident progress toward established goals.
  • Assist the nursing department with training of Restorative Aides.
  • Supervise Physical Therapy Assistants (PTAs) in direct resident care and resident-related activities in accordance with the applicable state practice act.
  • Communicate regularly with physicians, nursing staff, interdisciplinary team members, residents, and families regarding resident progress, goals, treatment plans, and discharge planning.
  • Participate in Resident Care Conferences, Utilization Review meetings, Rehabilitation meetings, and other interdisciplinary meetings as needed.
  • Provide in-service education and training programs for facility staff as assigned.
  • Complete timely, accurate, and compliant documentation in accordance with facility, governmental, and third-party payer requirements.
  • Maintain a safe treatment environment by following infection control, safety, and equipment maintenance protocols.
  • Educate residents, families, and caregivers regarding treatment goals, home exercise programs, maintenance programs, safety techniques, and fall prevention strategies.
  • Recommend and facilitate the ordering of appropriate durable medical equipment (DME) to promote resident independence and safety, including walkers, canes, splints, elevated toilet seats, grab bars, and other equipment as appropriate.
  • Maintain strict confidentiality of resident and company information.
  • Report problems with department equipment promptly to ensure equipment remains in safe and proper working order.
  • Contribute to quality assurance, performance improvement initiatives, and facility rehabilitation program development as assigned.
  • Establish and maintain positive, effective, and professional relationships with residents, families, facility staff, and professional colleagues.

Tice Valley Post Acute is seeking a compassionate and dedicatedPhysical Therapist (PT)to join our rehabilitation team. We are currently hiring for Full-Time, Part-Time, and PRN opportunities.

The Staff Physical Therapist is responsible for evaluating and treating residents, developing individualized treatment plans, monitoring progress, and collaborating with residents, families, physicians, nursing staff, and other members of the interdisciplinary team. The Physical Therapist will also provide education and in-service training to facility staff as needed.

General Purpose

The Physical Therapist evaluates and treats residents according to physician orders and facility policies, with the goal of improving mobility, strength, balance, endurance, safety, and overall functional independence.
