Job summary

Vacaville Ranch Post-Acute is seeking a Physical Therapist to provide rehabilitative care to residents, focusing on mobility, strength, balance and independence. The role collaborates with the rehabilitation team, interdisciplinary staff and physicians to support care planning, including supervision of PTAs and aides where applicable.

We offer a competitive hourly rate of $65-$70 based on experience, plus a $4,000 hiring and retention bonus for eligible full-time staff and opportunities for

Qualifications

• Active California Physical Therapist license in good standing.
• Bachelor’s, Master’s, or Doctorate degree in Physical Therapy.
• New graduates are welcome to apply; experience in skilled nursing or rehabilitation settings preferred.
• Knowledge of PDPM, Medicare, and skilled nursing facility regulations.
• Strong leadership, communication, and organizational skills.
• Commitment to resident-centered, high-quality care.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Implement evidence-based interventions to improve functional outcomes.
• Document resident progress accurately and in compliance with regulatory requirements.
• Collaborate with interdisciplinary teams and physicians.
• Supervise Physical Therapist Assistants and therapy aides, as applicable.
• Monitor clinical outcomes and contribute to departmental performance.
