Job summary

Vacaville Ranch Post-Acute is seeking an Occupational Therapist to design and implement resident-centered therapy programs that maximize independence, safety, and functional outcomes. You will collaborate with the rehabilitation team, document progress, and support families in care planning. New graduates are welcome; CA OT license required.

Prior SNF or rehab experience preferred. We offer full-time, part-time, PRN positions with a $4,000 hiring/retention bonus and competitive hourly pay of

Qualifications

• Active California Occupational Therapist license required.
• Bachelor's or master's degree in occupational therapy.
• New graduates welcome; experience in skilled nursing or rehabilitation settings preferred.
• Knowledge of PDPM, Medicare, and SNF regulations.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Implement evidence-based interventions to support functional independence.
• Document resident progress accurately and timely in accordance with regulatory requirements.
• Collaborate with interdisciplinary teams to optimize resident care.
• Participate in care conferences and family meetings as applicable.
• Mentor Occupational Therapy Assistants and support staff as applicable.
