Qualifications

• Active California Physical Therapist license in good standing.

• New graduates are welcome; experience in skilled nursing or rehabilitation settings preferred.
• Knowledge of PDPM, Medicare, and SNF regulations.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Implement evidence-based interventions to improve functional outcomes.
• Document resident progress accurately and in compliance with regulatory requirements.
• Collaborate with interdisciplinary teams and physicians.
• Supervise Physical Therapist Assistants and therapy aides, as applicable.
• Monitor clinical outcomes and contribute to departmental performance.
