Job summary

Concierge Home Care is seeking a Home Health Physical Therapist to deliver skilled therapy in patients’ homes, improving mobility and independence while aligning with the patient’s goals.

You will work with AI-assisted documentation and WellSky/Kinnser EMR tools to streamline care and documentation, with supportive leadership and flexible scheduling to enhance work-life balance.

Qualifications

• Active, unencumbered Florida Physical Therapist license.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Conduct patient evaluations and develop treatment plans.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on recovery and safety.
• Collaborate with physicians and interdisciplinary care teams.

Your Role as a Home Health Physical Therapist

As a Home Health Physical Therapist , you will provide skilled therapy services in the comfort of patients’ homes while helping them regain mobility, strength, and independence.

Responsibilities include:
