Job summary

Concierge Home Care is seeking a Home Health Physical Therapist to evaluate patients in their homes, design and implement mobility programs, and support recovery with patient and caregiver education. Your Role as a Home Health Physical Therapist

As a Home Health Physical Therapist, you will provide skilled therapy services in the comfort of patients’ homes while helping them regain mobility, strength, and independence.

Responsibilities include:

• Conduct patient evaluations and develop treatment plans
• Provide therapeutic exercises and mobility training
• Educate patients and caregivers on recovery and safety
• Collaborate with physicians and interdisciplinary care teams
• Supervise and support Physical Therapy Assistants (PTAs)
