Job summary

Concierge Home Care seeks a Home Health Physical Therapist to deliver skilled therapy in patients’ homes, helping regain mobility, strength, and independence.

You will plan and implement evaluations, develop individualized plans, and collaborate with care teams while using AI-assisted documentation tools to streamline visits. A Florida license is required; flexible scheduling, mileage and 1099 options available.

Qualifications

• Active, unencumbered Florida Physical Therapist license.
• Current driver’s license and reliable transportation.

Responsibilities

• Conduct patient evaluations and develop treatment plans.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on recovery and safety.

Your Role as a Home Health Physical Therapist
