Job summary

Concierge Home Care is seeking a dedicated Home Health Physical Therapist to deliver skilled therapy in patients' homes, helping restore mobility and independence with patient-centered care.

You will conduct evaluations, create treatment plans, and use AI-assisted tools to document visits, while collaborating with physicians and care teams to support optimal outcomes.

Qualifications

• Active, unencumbered Florida Physical Therapist license.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Conduct patient evaluations and develop treatment plans.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on recovery and safety.
• Collaborate with physicians and interdisciplinary care teams.

Your Role as a Home Health Physical Therapist

As a Home Health Physical Therapist, you will provide skilled therapy services in the comfort of patients’ homes while helping them regain mobility, strength, and independence.
