Job summary

Concierge Home Care is seeking a Contract Physical Therapist to provide skilled therapy in patients' homes, helping regain strength, mobility, and independence after illness or surgery.

As a contract clinician, you’ll manage visits, complete OASIS documentation, and collaborate with care teams while maintaining autonomy and flexible scheduling in a home health setting.

Qualifications

• Active, unencumbered Florida Physical Therapist license.
• OASIS documentation experience required.
• CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• Professional liability insurance.
• Workers’ Comp Exemption or Occupational Accident coverage.

Responsibilities

• Perform in-home physical therapy evaluations and treatments.
• Develop and implement individualized care plans.
• Complete Medicare OASIS documentation, including SOC, ROC, recertifications, discharges.
• Conduct evaluations.
• Educate patients and caregivers on mobility, safety, and recovery.
• Collaborate with physicians and interdisciplinary care teams.
• Supervise PTAs when applicable.

Your Role as a Contract Home Health Physical Therapist

As a Contract Physical Therapist, you will provide skilled therapy services in patients’ homes to help them regain strength, mobility, and independence following illness, injury, or surgery.

Responsibilities include:

• Perform in-home physical therapy evaluations and treatments
• Develop and implement individualized care plans
• Complete Medicare OASIS documentation, including:Start of Care (SOC), Resumption of Care (ROC), Recertifications, Discharges
• Evaluations
• Educate patients and caregivers on mobility, safety, and recovery
• Collaborate with physicians and interdisciplinary care teams
• Supervise Physical Therapy Assistants (PTAs) when applicable

Qualifications
