Job summary

Wilmington Nursing and Rehabilitation Center is seeking a Licensed Physical Therapist to provide direct patient care and develop individualized rehabilitation plans for residents in a long-term care setting. You will assess patients, design treatment programs, and work closely with the care team to optimize outcomes.

Ideal candidates are licensed PTs who are compassionate, self-motivated, and able to collaborate across disciplines.

Qualifications

• Must hold a valid physical therapy license in the state of practice.
• Experience in long-term care or geriatric settings preferred.

Responsibilities

• Provide direct patient care to assess medical condition, function, and rehab potential.
• Establish and administer treatment plans with goals based on patient capacity and physician direction.
• Document patient status and educate patients and families on therapy methods.
• Collaborate with other disciplines to deliver patient-centered care.

Physical Therapist (PT) - FT/PT/PRN Positions Available

$10K Retention Bonus is available for the FT Position.

Pay range that is posted on this position are dependent on status (FT/PT/PRN) and years of experience.

Skills of PTs: All qualified applicants should possess the following qualities:

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.
• Collaborates with all disciplines to provide best patient centered treatment and care.

Requirements/Qualifications:

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred.
