Job Responsibilities:

• Develop effective treatment plans and obtain approval for services from referring physicians.
• Treat patients according to the physician treatment plan.
• Assist nursing staff with training of Restorative Aides.
• Supervise Physical Therapy Assistants in direct patient care, following state practice acts.
• Communicate patient progress, issues, and plans with supervisors and team members.
• Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
• Conduct in-service training for staff.
• Document evaluations, treatment notes, progress notes, and recertifications per PT Board State Practice Act and payer requirements.
• Record treatment changes per policies and procedures.
• Instruct families and nursing staff in maintenance and caregiver training for discharge planning.

Minimum Requirements:

• Valid Physical Therapist License
