Physical Therapist, PT (Healthcare)

Job Responsibilities:

• Develop effective treatment plans and obtain approval from referring physicians.
• Treat patients according to the physician’s treatment plan.
• Assist the nursing department with training Restorative Aides.
• Supervise Physical Therapy Assistants in patient care, following state practice acts.
• Communicate with supervisors and healthcare team members regarding patient progress and plans.
• Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
• Conduct in-service training for staff.
• Document evaluations, treatment notes, progress reports, and recertifications as per PT Board State Practice Act and payer requirements.
• Record treatment modifications in accordance with policies.
• Educate patients’ families and nursing staff on maintenance and caregiver training for discharge planning.
• Participate in discharge planning processes.
• Order and recommend necessary durable medical equipment to support patient independence (e.g., braces, walkers, canes, etc.).
• Report equipment issues to ensure proper maintenance.
• Maintain positive and effective relationships with patients, families, staff, and colleagues.

Minimum Requirements:
