Job summary

AVOLA Therapy is hiring a Physical Therapist (PT) for the Phoenix West Valley, offering in-home patient care with new graduate mentorship and flexible scheduling.

You will evaluate and treat patients in their homes, develop personalized plans, and collaborate with physicians and interdisciplinary teams to ensure optimal outcomes.

Qualifications

• Licensed or license-eligible PT in Arizona.
• New graduates welcome and encouraged to apply.
• Home health experience is a plus, but not required — we'll train you.
• Reliable transportation for travel between patient homes.

Responsibilities

• Perform physical therapy evaluations and treatments in patients' homes across the Phoenix West Valley.
• Develop and implement individualized treatment plans across adult and pediatric populations.
• Address mobility, balance, strength, coordination, pain management, fall prevention, and motor development across the lifespan.
• Complete documentation and Medicare/OASIS as applicable.
• Coordinate care with physicians, home health agencies, and interdisciplinary teams.

What You'll Do

• Perform physical therapy evaluations and treatments in patients' homes across the Phoenix West Valley

• Complete OASIS assessments and Medicare documentation
