As a Home Health Physical Therapist supporting patients across the East Valley, you will deliver skilled, outcomes‑driven therapy in the home setting to help clients regain mobility, reduce pain, and maintain independence. Key responsibilities include completing comprehensive PT evaluations; creating and updating individualized plans of care aligned to physician orders; delivering therapeutic interventions and caregiver education; completing OASIS and all documentation on time in Homecare Homebase (HCHB); coordinating with the interdisciplinary team; supervising PTAs as assigned; and maintaining compliance with Medicare Conditions of Participation, agency policy, and Arizona regulations.

Your Monday–Friday schedule is organized around in‑home visits throughout the East Valley, with time built in to review referrals, confirm visit timing with patients and caregivers, and travel between homes. Between appointments, you’ll document in HCHB, check messages from the care team, and align next steps with the ordering physician when changes arise. You’ll also stay attentive to home setup and safety considerations, and you’ll close out the day by finalizing notes, planning the next route, and ensuring all items are completed within required timeframes.
This full‑time role offers a competitive salary range of $95,000–$135,000, plus Medical and Dental coverage and Paid Time Off to support rest and continuity of care.
