MyCHN Home Health Services is urgently hiring a highly skilled Physical Therapist PT to deliver coordinated care to patients across ages. Join a compassionate team dedicated to quality care in the community.

You will perform assessments, develop plans of care with physicians, supervise PTAs and HHAs, and travel to patient homes across Brazoria, Galveston, and Harris Counties. Requirements include a BS in PT, Texas PT license, and at least two years in acute care or rehab.

Qualifications

• BS in Physical Therapy from an accredited program.
• Current Texas State License as a Physical Therapist.
• Two years of acute care or rehabilitation experience.

Responsibilities

• Perform initial and ongoing clinical assessments to identify patient needs.
• Develop and revise a Plan of Care in consultation with the physician and care team.
• Provide therapeutic treatments and evaluate equipment needs.
• Evaluate outcomes of care with appropriate discharge planning.
• Apply infection control concepts to prevent contamination.
• Supervise PTA and HHA as appropriate.
• Participate in mandatory continuing education and in-service programs.
• Wound care management.

Required Education and Experience:

• Bachelor of Science in Physical Therapy from an accredited program.
• Current Texas State License as a Physical Therapist.
• Two years' experience in an acute care or rehabilitation setting.

Physical Requirements:

• Must be able to lift and/or move up to 50 pounds.
• Frequent talking, standing, and hand dexterity required.
• Close vision, color vision, and depth perception abilities needed.

Work Location & Travel Requirements:

• Home office location is MyCHN Home Health Services:4005 Technology Dr, Angleton, TX 77515, USA
• This position requires regular travel to patient homes and other assigned locations throughout Brazoria, Galveston, and Harris Counties.
