Job summary

MyCHN Home Health Services is seeking a dedicated Physical Therapist Assistant (PTA) to provide in-home physical therapy under the supervision of a licensed Physical Therapist in the Greater Houston area. You will help patients improve mobility, balance, strength, and independence while supporting recovery and quality of life.

Qualifications

  • Requires graduate of an accredited Physical Therapist Assistant program.
  • Current Texas PTA license in good standing is preferred as stated.
  • Strong communication, interpersonal, and patient education skills are essential.

  • Provide PT treatments and interventions per Plan of Care under supervising PT.
  • Assist with therapeutic exercises, gait training, balance activities, and mobility tasks.
  • Observe, document patient responses and report to supervising PT.
  • Identify changes in patient condition and communicate with care team.
  • Educate patients and families on home exercise programs and safety.

What You'll Do & Bring

As a Physical Therapist Assistant (PTA) you'll work under the supervision of a licensed Physical Therapist to provide physical therapy treatments and interventions in patients' homes. The PTA plays a vital role in helping patients improve mobility, strength, balance, endurance, coordination, and overall functional independence while supporting recovery and enhancing quality of life.

Key Responsibilities
  • Provide physical therapy treatments and interventions as outlined in the patient's Plan of Care established by the supervising Physical Therapist.
  • Assist patients with therapeutic exercises, gait training, balance activities, endurance training, functional mobility activities, and other prescribed treatments.
  • Observe, monitor, and document patient responses to treatment and communicate findings to the supervising Physical Therapist.
  • Recognize changes in patient condition and promptly report concerns to the Physical Therapist and interdisciplinary care team.
  • Educate patients, families, and caregivers on prescribed therapeutic exercises, home exercise programs, safety precautions, and techniques to support recovery and independence.
  • Reinforce patient goals and encourage adherence to treatment plans.
  • Assist with the evaluation and recommendation of durable medical equipment (DME), adaptive devices, and assistive equipment as directed by the Physical Therapist.
  • Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure coordinated patient care.
  • Maintain accurate, complete, and timely clinical documentation in accordance with agency policies and state and federal regulatory requirements.
  • Apply infection control practices and promote patient safety during all visits.
  • Participate in required continuing education, in-service training, and professional development activities.
What You Bring
  • Graduate of an accredited Physical Therapist Assistant program.
  • Current Texas Physical Therapist Assistant license in good standing.
  • Strong knowledge of accepted physical therapy practices and treatment techniques.
  • Ability to work effectively under the direction and supervision of a licensed Physical Therapist.
  • Excellent communication, interpersonal, and patient education skills.
  • Strong organizational, time-management, and clinical documentation skills.
  • Knowledge of durable medical equipment, adaptive devices, and assistive technology.
  • Ability to independently manage a patient caseload and prioritize responsibilities.
  • Passion for providing compassionate, patient-centered care.
  • Ability to work a flexible schedule and travel throughout assigned service areas.
  • Bilingual English/Spanish preferred.
  • Preferred:
  • Bilingual in Spanish
  • Current CPR or Basic Life Support (BLS) certification preferred.
  • Home health experience is strongly preferred.
Work Location & Travel Requirements:
  • Home office location is MyCHN Home Health Services:4005 Technology Dr, Angleton, TX 77515, USA
  • This position requires regular travel to patient homes and other assigned locations throughout Brazoria, Galveston, and Harris Counties.
