Job summary

MyCHN Home Health Services in Texas is seeking a Physical Therapist to deliver high-quality in-home rehabilitation. You will assess, plan, and implement personalized therapy programs to restore mobility and independence in patients’ homes.

You will educate families, coordinate with an interdisciplinary team, and document care while traveling within Brazoria, Galveston, and Harris Counties. A Texas PT license and BLS are required, with bilingual skills and home-health experience preferred.

Qualifications

  • Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
  • Current Texas Physical Therapist license in good standing.
  • Current Basic Life Support (BLS) certification.
  • Strong therapy skills based on accepted standards of physical therapy practice.
  • Excellent assessment, clinical decision-making, and problem-solving abilities.
  • Strong interpersonal, communication, and patient education skills.
  • Ability to work independently while effectively collaborating with an interdisciplinary care team.
  • Strong organizational and documentation skills.
  • Ability to work a flexible schedule and travel within assigned service areas.
  • Reliable transportation and current automobile liability insurance.
  • Commitment to delivering compassionate, patient-centered care.
  • Preferred: Bilingual in Spanish
  • Preferred: Home health experience

Responsibilities

  • Perform initial and ongoing clinical assessments to identify patient needs and functional limitations.
  • Evaluate patients' physical condition, mobility, strength, balance, endurance, and overall functional abilities in the home setting.
  • Develop, implement, and revise individualized Plans of Care in collaboration with physicians and the interdisciplinary care team.
  • Provide skilled physical therapy services, including therapeutic exercises, gait training, transfer training, balance training, fall prevention education, and functional mobility interventions.
  • Educate patients, families, and caregivers regarding treatment plans, therapeutic techniques, safety precautions, and home exercise programs.
  • Monitor patient progress, evaluate treatment effectiveness, and modify care plans as needed to achieve established goals.
  • Evaluate outcomes of care and coordinate appropriate discharge planning.
  • Provide wound care management as appropriate within the scope of practice.
  • Apply infection prevention and control principles to maintain a safe environment for patients and caregivers.
  • Maintain accurate, complete, and timely clinical documentation in accordance with agency policies and state and federal regulations.
  • Collaborate with physicians, nurses, therapists, home health aides, and other healthcare professionals to coordinate patient care.
  • Supervise Physical Therapist Assistants (PTAs) and Home Health Aides (HHAs) as appropriate.
  • Participate in case conferences, quality improvement initiatives, mandatory continuing education, and in-service training programs.

What You'll Do & Bring

As a Physical Therapist (PT), you will play a vital role in helping patients regain strength, mobility, function, and independence while receiving care in their homes.
