Workstream is urgently hiring a highly skilled Physical Therapist in Angleton, Texas, to provide coordinated care to patients of all ages. Responsibilities

• Perform initial and ongoing clinical assessments to identify patient needs
• Develop and revise a Plan of Care in consultation with the physician and care team
• Provide therapeutic treatments and evaluate equipment needs
• Evaluate outcomes of care with appropriate discharge planning
• Apply infection control concepts to prevent contamination
• Supervise PTA and HHA as appropriate
• Participate in mandatory continuing education and in-service programs
• Wound care management

Skills

Strong therapy skills based on accepted standards of practice

Excellent interpersonal skills

Knowledge of durable medical equipment

Ability to work a flexible schedule and travel locally

Reliable transportation and valid auto liability insurance

Education

Bachelor of Science in Physical Therapy from an accredited program
