Job summary

PurposeCare is seeking a licensed Physical Therapist to provide home health services with flexible scheduling and autonomy in patient care. You will assess, plan, and deliver therapeutic interventions in clients' homes, coordinating with families and care teams to support independence.

Qualified candidates hold a current PT license in the applying state, with 1 year of PT experience preferred (home health and OASIS a plus), and a valid driver's license. EOE.

Qualifications

• Current Physical Therapist license in the state you are applying in.
• 1 year of physical therapist experience is preferred - home health and OASIS experience preferred.
• Ability to perform tasks involving physical activity, up to 50 lbs, bending and standing.
• Current Driver's License, vehicle insurance, and/or access to dependable transportation.
• Ability to successfully complete a background check.
