Job summary

PurposeCare is seeking a licensed Physical Therapy Assistant to join our home health team in the Midwest. You will provide skilled visits in patients' homes, support recovery goals, and coordinate with caregivers to maintain quality care.

The role emphasizes autonomy, personalized patient connections, and practical therapy that improves daily living. A current PTA license and driving access are required, with a compassionate, patient-centered approach encouraged.

Qualifications

• Current Physical Therapy Assistant license in the state where applying.
• 1 year of physical therapy assistant experience preferred (home health experience preferred).
• Ability to perform tasks involving physical activity, including heavy lifting up to 50 lbs, bending, and standing.

Responsibilities

• Provide skilled physical therapy visits in patients' homes.
• Support patient recovery goals and daily living activities.
• Coordinate with caregivers and care teams to ensure quality care.

Qualifications and Requirements:

• Current Physical Therapist Assistant license in the state in which you are applying
• 1 year of physical therapy assistant experience is preferred - home health experience preferred
• Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing
• Current Driver's License, vehicle insurance, and/or access to dependable transportation
• Ability to successfully complete a background check
