PurposeCare is a leading home health provider, seeking a licensed Physical Therapist to join our Ohio-based team. This role involves delivering in-home physical therapy services, coordinating care with patients in their own homes, and contributing to a patient-centered care plan.

Qualifications

• Current Physical Therapist license in the state in which you are applying.
• 1 year of physical therapist experience preferred (home health and OASIS experience preferred).
• Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing.
• Current Driver's License, vehicle insurance, and/or access to dependable transportation.
• Ability to successfully complete a background check.
