Job summary

Ultimate Therapy is hiring a Physical Therapist (PT) to provide rehabilitation services at Autumn Lake at Norwalk. This full-time role supports residents in restoring movement, increasing strength, and achieving functional goals.

You will perform evaluations, establish plans of care, treat mobility and balance issues, and coordinate with PTAs, OTs, SLPs, nurses, and other professionals. CT license and accredited PT program graduate required; new grads welcome.

Qualifications

• Current Connecticut Physical Therapist license required.
• Graduated from an accredited Physical Therapy program.

Responsibilities

• Perform comprehensive physical therapy evaluations and establish individualized plans of care.
• Provide skilled treatment addressing mobility, strength, balance, gait, transfers, and endurance.
• Establish functional goals and track resident progress throughout the rehabilitation process.
• Modify treatment plans based on clinical findings and response to therapy.
• Educate residents and caregivers on exercises, mobility techniques, and fall-prevention strategies.
• Complete required clinical documentation accurately and on time.
• Coordinate care with PTAs, OTs, SLPs, nursing staff, and other healthcare professionals.

Ultimate Therapy is hiring a Physical Therapist (PT) to provide rehabilitation services at Autumn Lake at Norwalk. Join a supportive clinical team focused on helping residents restore movement, increase strength, and make meaningful progress toward their functional goals.

What You'll Do
