Job summary

New Vista Nursing and Rehabilitation Center in New Jersey is seeking a licensed Physical Therapist for a full-time role in a sub-acute/long-term care setting. You will evaluate, plan, direct and administer PT treatments as prescribed by a physician.

The position requires at least 2 years of experience in skilled nursing, NJ license, day shift with additional PRN coverage Monday through Saturday; remote work is not available.

Qualifications

• New Jersey Physical Therapy license required.
• Minimum 2 years' experience in Skilled Nursing Facility.
• Experience supervising staff and coordinating care preferred.

Responsibilities

• Evaluate, plan, direct and administer physical therapy modalities as prescribed by a licensed physician.
• Administer treatments and physical agents to restore function and prevent disability.
• Assist patients in reaching maximum potential within their capabilities.
• Supervise, coordinate, and delegate responsibilities to supportive staff.
• Ensure accurate, timely documentation of patient changes.
• Maintain infection control practices and proper PPE in designated rooms.

Full Job Description

• Setting: Sub-Acute/Long-term Care setting.

• Responsible for evaluation, planning, directing, and administering physical therapy modalities of treatment as prescribed by a licensed physician.

• Administers treatments and physical agents as prescribed by a referring physician in an effort to restore function and prevent disability following injury, disease, or physical disability.

• Assists in helping patients reach their maximum potential and learning how to work within their capabilities.

• The staff therapist will be responsible for supervising, coordinating, and delegating responsibilities to the supportive staff.

• Ensures patient changes are documented accurately and on time.

• Responsible for maintaining and helping to enforce any infection control with cleaning equipment and wearing proper PPE in PUI rooms.

• Must have at least 2 years experience in Skilled Nursing Setting.
