Job summary

The Per Diem Physical Therapist at Jewish Home Family will perform screenings, evaluations, re-evaluations and discharges under physician supervision, and will supervise Physical Therapy Assistants as directed by the Director of Rehabilitation.

Key duties include developing a Plan of Care, reassessing every 30 days, quarterly patient screening, discharge planning, progress documentation in the EMR, and collaboration with the care team to maximize resident independence.

Qualifications

• Graduate of an accredited physical therapy program.
• Current NJ state license in Physical Therapy.
• Maintain state and national licensure requirements.
• CPR certified.

Responsibilities

• Screen all patients quarterly to identify decline in function.
• Discharge planning to maximize independence in least restrictive environment.

• Coordinate staff in-services to ensure safe carryover of programs.

Job Summary

PER DIEM - The Physical Therapist will complete screening, evaluations, re-evaluations, and discharges as supervised by the Physician. The Physical Therapist will provide supervision to Physical Therapy Assistants as directed by the Director of Rehabilitation.

Responsibilities and Duties

• Evaluates patients assigned per MD orders and as indicated, and establishes an appropriate Plan of Care and Goals that meet the specific needs of the individual.
• Re-assesses patients and completes Updated Plan of Care at least every 30 days, or as indicated on initial evaluation, in order to determine the need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated.
• Screens all patients in facility on a quarterly basis, to identify those who have experienced declines in their functional abilities and determine if Physical Therapy Intervention is required.
• Provides thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment.
• Observes and records patient’s progress, response to treatment and limitations and maintains this information in electronic medical record.
• Completes required documentation at point of service.
• Supervises Physical Therapy Assistants as assigned by the Director of Rehabilitation.
• Attends and reports accurately a patient’s status at weekly UR meeting and interdisciplinary care plan meeting.
• Identifies needs for program development and implement programs according to company policies and procedures.
• Coordinates in-services for facility staff as need is determined, to ensure safe and effective carry over of Restorative Programs and overall call of the residents.
• Takes a patient centered approach to provision of care.
