Job summary

Step Up Rehab, LLC is seeking a licensed Physical Therapist (PT) in Florida to deliver skilled therapy within SNFs at the Milton and Century locations. This role focuses on restoring function and independence for residents through collaborative care in a SNF setting.

We offer a full-time, on-site position with hourly pay, a sign-on bonus of $10,000, and a comprehensive benefits package including 401(k), PTO, and health insurance options.

Qualifications

• Graduate of an accredited PT program.
• Active Florida PT license (or license-eligible) required.
• SNF or post-acute experience preferred.
• Net Health knowledge preferred.

• Provide skilled PT-related therapy services to residents in a Skilled Nursing Facility while collaborating with the interdisciplinary team to improve safety, function, and quality of life.

Step Up Rehab is the in-house therapy provider at Santa Rosa Center for Rehabilitation and Healing and Sandy Ridge Center for Rehabilitation and Healing.

Work inside the dedicated therapy gyms within these skilled nursing facilities, helping residents regain function and independence through skilled, compassionate care.

Physical Therapist (PT) – Skilled Nursing Facility (SNF)

This position is exclusively for professionals licensed in the state of Florida as a PT. All services are provided in a Skilled Nursing Facility (SNF).
