Rehab Advisors, part of Enhance Therapies family, is hiring a Physical Therapist (PT) to join an in-house therapy program in a skilled nursing/long-term care setting. The role emphasizes patient evaluation, treatment planning, and collaborative care with a focus on outcomes.

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional impairments.
• Create and implement personalized care plans.
• Collaborate with team to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all physician orders are obtained and up to date.
