Job summary

IHA is seeking a licensed Physical Therapist to join our Michigan team in Ludington. The PT will assume autonomous responsibility for evaluating patient needs and delivering therapy within scope, with a day shift in a full-time schedule.

Responsibilities include evaluation, diagnosis, and goal setting with patient/family, plus documentation per standards. Collaborative, interdisciplinary care is emphasized, with a mission to provide high-quality, patient-centered care.

Qualifications

• Graduate of a Doctorate’s or equivalent PT training program accredited by the APTA.
• Bachelor’s degree in Physical Therapy.
• FSBPT Certification.
• Michigan PT License or eligibility before hire.
• AHA BLS or equivalent.

Responsibilities

• Provide treatment and services within the scope of their license and as required by law/regulation.
• Evaluate and clinically diagnose patient conditions.
• Establish treatment goals with input from patient and family; document per standards.
• Participate as part of an interdisciplinary, patient-centered care team to integrate a PT plan.

What The Physical Therapist ( PT ) Will Do

• Providing treatment and services within the scope of their license, certification or registration and as required by law and regulation.
• Evaluation and clinical diagnosis of patient conditions.
• Establish treatment goals utilizing input from patient and their family as appropriate Documentation remains in accordance with established internal/external timeliness standards, insurance standards, and reimbursement guidelines.
• Participates as a part of an interdisciplinary and patient-centered care team which includes the verbal and written exchange of information with other healthcare team members as it pertains to integrate a physical therapy treatment plan

Minimum Qualifications

• Must be a graduate of a Doctorate’s or previously equivalent professional Physical Therapy training program accredited by the American Physical Therapy Association.
• Required: Bachelor’s in Physical Therapy
• FSBPT Certification.
• State of Michigan Physical Therapist License or eligible to obtain licensure prior to hire date.
• American Heart Association BLS or equivalent
